Our Transitional Care Team works to reduce readmissions and improve patient outcomes. During your stay at Trinity Medical our Transitional Care Team will work with you and your family from the beginning of your stay through your discharge to insure that you have appropriate follow up care. This may include home health, hospice, durable medical equipment, nursing facility placement and more.
